Legal Guardianship Requirements

If the student lives in the District with someone other than the student’s parent, the following must be provided to the District:

        1. A notarized letter, Power of Attorney or Authorization Agreement for NonParent Relative or Voluntary Caregiver from the student’s parent or other person having lawful control under a court order assigning responsibility for the student in all school-related matters to an adult resident of the District, and

        2. Acknowledgement to Investigate a Power of Attorney, and

        3. Legal Guardianship Agreement - The adult resident assuming responsibility for the student must complete, notarize and submit a notarized Legal Guardianship Agreement, and

        4. A copy of a valid government issued Identification, and

        5. Proof of Residence - A current electric bill (within 45 days), water bill (within 45 days) or lease agreement with printed information including the service address and the legal guardian’s name.

        6. Report cards - Elementary 1 year, Secondary students must submit the two previous school years of report cards.


Requisitos de tutela legal

Si el estudiante vive en el Distrito con alguien que no sea el padre, tutor u otra persona que tenga el control legal bajo una orden judicial, se debe proporcionar al Distrito lo siguiente:

      1. Un documento notariado (carta, poder, o acuerdo de autorización para la responsabilidad o control legal del estudiante bajo una order judicial de un cuidador voluntario o familiar que no sea el padre u otra persona) que asigne la responsabilidad del estudiante en todos los asuntos relacionados con la escuela a un adulto residente del distrito. y

      2. Reconocimiento para investigar un poder notarial, y

      3. Acuerdo de tutela legal: el residente adulto que asume la responsabilidad del estudiante debe completar, certificar y presentar un Acuerdo de tutela legal notariado, y

      4. Una copia de una identificación válida emitida por el gobierno, y

      5. Comprobante de residencia: una factura de electricidad actual (dentro de los 45 días), factura de agua (dentro de los 45 días) o contrato de arrendamiento con información impresa que incluye la dirección del servicio y el nombre del tutor legal.

      6. Boletas de calificaciones - 1 año de primaria, los estudiantes de secundaria deben presentar las boletas de calificaciones de los dos años escolares anteriores.

Revocation of Legal Guardianship Agreement or Power of Attorney

    • Guardians can revoke a Legal Guardianship Agreement by completing and submitting a Notice of Revocation form to the campus or the Office of Admissions.

    • Parents can revoke a Power of Attorney or an Authorization Agreement for NonParent Relative or Voluntary Caregiver by completing and submitting a Notice of Revocation form to the campus or the Office of Admissions.

      1. The parent or guardian must present a valid driver’s license or other acceptable picture ID.
