Proof of Residence Requirements
At the time of registration, one of the following forms of expedited proof of residency must be provided to the District in the form of one of the following.
STUDENTS LIVING WITH PARENT IN ANOTHER ADULT’S RESIDENCE (Multiple Families living in the same household) This form must be updated annually.
If the student lives with a parent or guardian at a residence within the district boundaries that is owned or rented by an adult other than the parent or guardian, the following proof of residency must be uploaded to the online registration parent portal:
The parent/guardian must complete, notarize, and upload an Affidavit of Residency Statement (Declaracion De Residencia Notariada) to the online registration parent portal.
STUDENTS LIVING WITH PARENT IN ANOTHER ADULT’S PROPERTY (Multiple households within the same property). This form must be updated annually.
If the student lives with a parent or guardian at a residence within the district boundaries in which an adult other than the parent owns the property or guardian, the following must be uploaded to the online registration parent portal:
The parent/guardian must complete, notarize, and upload an Affidavit of Residency Statement (Declaracion De Residencia Notariada) to the online registration parent portal.
STUDENTS LIVING APART AND SEPARATE (Legal Guardians/Tutors)
If the student lives in the District with someone other than the student’s parent, guardian, or other person having lawful control under a court order, the following must be provided to the District:
1. A notarized letter, Power of Attorney or Authorization Agreement for NonParent Relative or Voluntary Caregiver from the student’s parent or other person having lawful control under a court order assigning responsibility for the student in all school-related matters to an adult resident of the District; and
2. The adult resident assuming responsibility for the student must complete, notarize and submit a Legal Guardianship Agreement online or in person at the Office of Admissions (649 Webster St.), which will require presentation of a valid driver's license or other acceptable picture ID and:
The most recent (within 45 days) light or water bill with printed information including the service address and the adult’s name; or
A current lease agreement.
Revocation of Legal Guardianship Agreement or Power of Attorney
Guardians can revoke a Legal Guardianship Agreement by completing and submitting a Notice of Revocation form to the campus or the Office of Admissions.
Parents can revoke a Power of Attorney or an Authorization Agreement for NonParent Relative or Voluntary Caregiver by completing and submitting a Notice of Revocation form to the campus or the Office of Admissions.
The parent or guardian must present a valid driver’s license or other acceptable picture ID.
